Independent School District-Teacher Retirement (ISD TRS) Plans

We at Village Health Partners greatly value the trust that you have placed in us to provide high quality care and the personal attention you deserve. Village Health Partners was recently notified that January 1, 2017 the Independent School District-Teacher Retirement (ISD TRS) is changing insurance plans to Humana Medicare PPO or HMO.  These plans are not in network with Village Health Partners.

The Humana Medicare PPO plan that is being offered will allow you to continue to see your Village Health Partner physician out of network.

Unfortunately, future care from Village Health Partners will not be covered if you select the Humana Medicare HMO plan.

Please contact your insurance provider for more information about your family medicine coverage options on your current plan.
