Immigration Exams in Plano, TX

At Village Health Partners, we have your family covered from basic medical tests and physicals to wound care and minor procedures. We are a family practice provider and can cover the majority, if not all, of your healthcare needs, including immigration exams for men, women, and children entering and residing in the U.S. from another country. Our physicians are certified by USCIS, formerly INH to perform your immigration exam. The immigration medical exam must be conducted by a physician who is designated by USCIS (also known as a civil surgeon). We are here to provide thorough, accurate, and affordable care to clients and will ensure that your exam is expedited quickly, meeting all requirements and government protocols.

Our physicians at Village Health Partners are registered to provide INS physical examinations, immunizations, and other healthcare-related services as governed by the United States Citizenship and Immigration Services (USCIS). We welcome you to make an appointment or call for a walk-in appointment to have your immigration exam completed. Before arriving at our medical facility for your exam, please make sure you bring the following:

• A valid picture government ID (driver's license or passport, cannot be expired)
• I-693 paperwork (download at uscis.gov)
• Official shot record (without proof of immunizations, you must receive all required vaccines)

We recommend that you print out and bring the most recent version of Form I-693, Report of Medical Examination and Vaccination Record, to your medical exam appointment at Village Health Partners. Please carefully fill out the paperwork accordingly, completing Part 1 of the I-693 form, but do not sign your name until our medical team instructs you to do so. This form will be used to document and detail the results of your immigration medical exam.

Once the exam has been completed, our staff at Village Health Partners, under the guidance of our civil surgeon, will complete, sign, and seal the official I-693 form, along with any supporting documents in an envelope. It is the patient's responsibility to submit this sealed envelope to USCIS. This information is documented on the I-693 form under instructions.

Immunization requirements at Village Health Partners are as follows:
Please note that we CANNOT place your Tuberculosis (TB) skin test within 30 days of any live vaccines, such as MMR, Varicella, or a TB skin test. If you have had any of these vaccines or had a TB skin test recently, you must wait 30 days to have your immigration exam. The TB skin test MUST be placed and read by the civil surgeon doing the immigration physical. If you have had a positive TB skin test in the past, we must have documented proof, including the date it was placed, read, and the size of the reaction. Without this documentation, we must place the TB skin test again.

If the applicant is between the ages of birth – 17, you need:
The child must be up-to-date based on our immunization standards. We will not be able to let you know what the child requires until you're in our office, and we have the shot record in front of us to review.

If the applicant is between the ages of 18 – 64, you need:

• Tdap vaccine
• Varicella vaccine or history of varicella illness (chicken pox)
• MMR (if born after 1957)

If the applicant is 65 years of age and above, you need:

• Tdap vaccine
• Pneumococcal vaccine
• Varicella vaccine or history of varicella illness (chicken pox)
• During flu season (December 1 to March 31): ALL applicants 6 months old to 18 years old and 50 years old and above are required to have an influenza vaccine during that season.

Additional test requirements:

• Blood test for TB: Required for applicants 2 years of age and above
• Syphilis test: Required for applicants ages 15 and above
• Gonorrhea Urine test: Required for applicants ages 15 and above
• Pregnant women can’t receive certain live vaccines, and they should bring a letter from their OB/GYN stating their pregnancy status
