Our blended model of care
&srotate=0)
The coronavirus has changed the way we live, learn, work and socialize – but it hasn’t changed our commitment to providing you with accessible, convenient and quality care.
We’re doing this through a blended model of care which includes telehealth visits, curbside care and in-person appointments.
Telehealth
Telehealth visits with your physician work similarly to in-person visits. Your physician will review your medical history and symptoms and provide you with a treatment plan, including prescribing medication if needed. These visits are done through our Patient Portal.
What can you do through telehealth? Here are some things we can see you virtually for:
- Cold and flu symptoms
- Allergies
- Medication refills
- Follow-up visits
- Rashes
- Dietitian visits
- Counselor visits

Curbside care
Our transition to telehealth has ensured you can continue to see your physician, counselor or dietitian while feeling safe and comfortable at home. And while we can do many things through telehealth, we can’t provide testing through our virtual visits.
Our drive-through testing site has been expanded to provide care for patients even when you can’t come into the office.
Here’s what tests we are currently providing at our drive-through site:
- Flu tests
- Strep tests
- Hemoglobin A1C tests
All you need is a telehealth visit. If it’s determined that you need a flu test, strep test, or a Hemoglobin A1C test that cannot be completed virtually, you’ll receive a referral for drive-through care.

In-person
We are currently seeing patients in-office! Find out what to expect here.
